Objective Structured Clinical Examination(OSCE)- What is it?

Objective Structured Clinical Examination (OSCE) is a widely used method for assessing clinical competence in healthcare education, including physiotherapy. It is a practical exam used in physiotherapy education to test clinical and professional skills in a very organized way.

A performance-based assessment in which physiotherapy students rotate through a series of stations, where they are required to perform specific clinical tasks within a set timeframe under observation. The OSCE is a type of practical exam used in physiotherapy to test how well students can do real-life clinical tasks, like checking joints, talking to patients, or planning treatment.

Purpose of OSCE in Physiotherapy

OSCEs aim to:

  • Assess clinical competencies in a controlled environment.
  • Evaluate both cognitive (knowledge), psychomotor (skills), and affective (communication and professionalism) domains.
  • Provide standardized and reproducible assessments of practical skills.
  • Identify strengths and weaknesses for further training.

A typical OSCE includes multiple stations, each lasting 5–10 minutes, designed to assess specific competencies.

Station Types:

  1. Practical Skill Stations
    • E.g., Joint mobilization, gait assessment, muscle testing, application of electrotherapy.
  2. History Taking Stations
    • Interviewing a simulated patient to gather subjective data.
  3. Clinical Reasoning Stations
    • Interpreting findings and planning treatment.
  4. Communication Stations
    • Explaining diagnoses, giving advice, or demonstrating empathy.
  5. Documentation Stations
    • Writing SOAP notes or treatment plans.
  6. Professionalism/Ethics Scenarios

👥 Examiners and Standardized Patients:

  • Examiners use a structured checklist or rubric to assess the candidate.
  • Standardized Patients (SPs) (actors trained to simulate conditions) are often used to create realistic scenarios.

Assessment is typically done using:

  • Checklists: Specific steps of the procedure.
  • Global Rating Scales: Overall competence, safety, confidence, communication, etc.
  • Time Management: Completing tasks within the given period.
  • Professional Behavior: Dress code, ethics, patient interaction.

Scoring and Feedback

  • Each station is scored independently.
  • Scores may be numerical (points for steps done correctly) and/or qualitative (excellent, good, needs improvement).
  • Immediate or post-exam feedback helps students understand their performance.
